Question:

What is the ruling in Islam about Muslims who are mentally ill but functional in society, and their illness prevents them from adhering to the deen due to paranoia, delusions, etc? Do we hold them accountable for haraam actions? Are they accountable before Allah?

Zaynab El-Kateb:

If they are not able to realize and distinguish between halal and haram, then Allah will not make then accountable for their deeds in this world and they will be tested on the Resurrection Day.
